PIA Request Forms

In accordance with Policy No. MDOT 060, the attached forms shall be available on the MDOT internet site for all MDOT employees and the public to use when submitting MDOT PIA requests.

Applicants may request documents for review or reproduction online, in writing, in person, by phone, and by email. All requests, regardless of the form in which they are made, should explicitly state that the documents are being requested under the PIA. If someone is requesting information or those words are not included, the request may be made but the PIA mandated requirements may not apply.

To facilitate PIA requests within the response times required by law, MDOT encourages the public and employees to use the appropriate attached forms available online when possible, andclearly state that the documents are being requested under the PIA. When a form is submitted through the internet site, the appropriate MDOT staff will receive the request by email.

These documents may be revised, as required, with the approval of the Deputy Secretary of Policy, Planning, and Enterprise Services. Once approved, the Office of Transportation Technology Services shall remove the old form(s) and replace with the new, approved version(s)
